passenger wiper arm keeps coming off

when using the wiper system the passenger wiper arm will disconnect and lay at bottom of windshield the driver side still works if the wiper is at bottom but sometimes it is caught over the passenger side wiper arm

  The wiper arm may be loose from their wiper moving shaft. Gently, remove the cap and tighten the 15 mm nut.

  is there suppose to be a clip or piece inside the wiper arm shaft as when I look at this it is a nut inside a circular area. Nothing to adjust it just fits over the top and use pliers to press back into place but it will work its way back off when wiper is in use again ( when I removed the cover over the assembly there is no cap over the nut?)
