CVO 10/7/2012I'm assuming, it must be the cooling fan motor. See this FIG : Circuit Diagram (2004)

Check the cooling fan relay.
1.Reset it by pulling the relay out of their socket and if it stops the fan, replace it.
2. In the event, if it doesn't work, check the fan control module. -

dagerm89 10/29/2012Do you mean the engine fan (at the front of your van) or the fans inside of the cabin? If it is the engine fan, it is a common issue with MPV's and Mazda6's which requires that the fan control module needs to be replaced. The part itself runs anywhere from $120ish to $150ish last time I checked.
My Mazda6's engine fan would continue to run even after I turned off the car and pulled the keys from the ignition, and once I had it replaced I have had no issues with it.
